Instructions for a social work consult

- Continue to use tasks to communicate about patients that have been previously referred

- Cooking Matters
- Diana Pantalos, metabolic nutritionist at WCEC, has received a grant to offer grocery store tours to low income families at certain stores in downtown and west Louisville. Topics covered during the 1.5 hour tour include reading food labels, locating whole grain foods, and identifying different ways to buy produce. The tour ends with an opportunity for parents to take home $10 worth of healthy food that they select.
- To qualify: must be on WIC or have SNAP benefits, and children must be newborns to 5 years of age.

- In addition to many other programs, offers Play Dates in which everyone gets to have fun, and also parents parents are given a chance to participate in parenting discussions. Positive parenting strategies are demonstrated by the organizers.
Resources for families in need of home visits for care of infants and toddlers

- Can enroll at any time, from prenatally (preferred) up until the child is 18 months old
- Home visits are provided monthly, and rotate between a Resource Worker (monthly) and an RN (quarterly)

- DVIC is open 24/7.
- Victim can take out both an Emergency Protective Order (EPO/TIPO) and/or a criminal complaint at the same time.
- Who can get one:
- Domestic Violence Orders (DVO): A spouse, former spouse, a member of a couple who lives together or formerly lived together, a grandparent, a parent, a child, a stepchild or any other person living in the same household as a child (if the child is the alleged victim).
- Interpersonal Protective Orders (IPO): Unmarried couples (such as teens, college students, elderly couples) can also get protective orders.

- Pivot to Peace - a hospital-based program aimed at preventing young adult violence, and improving community capacity to address all forms of violence. Consider referring adolescents who have been the victim of peer or gang violence.

- Provides food through many partner organizations. Parents can visit the website or call Dare to Care. The website is generally the easiest way to find a pantry close by. There is an annual limit at each organization, but using up your limit at one site does not prevent you from going to another site.
