Patients who Qualify for Synagis
Bronchopulmonary Dysplasia/Chronic Lung Disease
Specific Criteria:
Received medical treatment for BPD/CLDP within 6 months of start of RSV season, including:
- Supplemental Oxygen
- Bronchodilator
- Diuretic
- Corticosteroid therapy
Birthday Criteria:
Born after October 31, 2014
Congenital Heart Disease
Specific Criteria:
Disease that is hemodynamically significant as defined by (any 1 of these):
- Receiving medication to control congestive heart failure
- Moderate to several pulmonary hypertension
- Has cyanotic heart disease
Birthday Criteria:
Born after October 31, 2014
Premature
≤28 weeks, 6 days GA if born after October 31, 2015
29 weeks, 0 days to 31 weeks, 6 days GA if born after April 30, 2016
32 weeks, 0 days to 34 weeks, 6 days GA if born after July 31, 2016 and either one of:
- Attends daycare
- Lives in the same household as another child <5 years of age
